Job Description
The Family Connections Case Aide assists families in the pursuit of preventive services, providing support through intake, assessment, goal setting, and case management. This role involves collaboration with a team, adhering to a strong fidelity model, and providing outreach to connect families with resources. The position emphasizes a supportive and dynamic work environment focused on providing evidence-based practice.
Requirements
- High school diploma or GED
- 1-2 years working in Preventive Services or fast case setting
- Seeking candidates bilingual in Spanish
- Some evening hours required
- Excellent organizational, written and oral skills
- Proficient in Microsoft Office
- Commitment to working from a strength based perspective
- Must be able to travel
- Strong writing and communication skills
