Job Description
Atlantic Health System is seeking a Per Diem Social Worker for Case Management at Chilton Medical Center. The social worker will provide discharge planning support, resources, referrals, transfers to Extended Care and inpatient psychiatric facilities. Work closely with the Healthcare Team to implement the plan of care, obtain insurance authorizations as required, communicate with community resources and area facilities.
Requirements
- Provide Social Work coverage to facilitate discharges
- Meet with patients and families to assess and provide discharge planning support
