Case Manager

The Case Manager will provide community-based coordination and assistance to individuals with mental health needs, focusing on their ability to succeed in community. Responsibilities include facilitating service access, providing ongoing assessment and support, and coordinating individualized service plans. The role requires excellent interpersonal and organizational skills.

Requirements

• Provide specific, measurable, and individualized services to clients.
• Identify and access needed services.
• Facilitate daily living skills development.
• Conduct symptom monitoring and crisis management.
• Maintain knowledge of F&CS policies and procedures.
• Ensure compliance with OHMAHS laws and regulations.
• Maintain current knowledge of federal, state, and local laws.
