Job Description
This position is for a Substance Abuse Counselor to provide medically assisted treatment and support patients through opioid addiction. The counselor will facilitate individual and group therapy, manage clinical and outpatient services, and collaborate with physicians and other healthcare professionals. They will work to achieve positive patient outcomes through a combination of counseling, case management, and care planning.
Requirements
- Conduct individual initial assessments and evaluations.
- Facilitate patient treatment plans.
- Complete Bio-psychosocial assessments.
- Provide individual and group counseling services.
- Coordinate all services throughout treatment.
- Monitor patient progress.
- Develop aftercare plans and discharge plans.
