Job Description
St. Gertrude's is seeking a Minnesota licensed Social Worker to provide social service assessments, care planning, resident advocacy and relationship building with patients in our Transitional Care Unit.
Requirements
- Current social license
- 1 + years' experience in Skilled Nursing Facility
- Familiar with federal and state LTC regulations
- CPR certified
- Excellent verbal and written communication skills
- Able to read, write and communicate in English
