Patient Care Coordinator

Patient Care Coordinator will work collaboratively with the multidisciplinary team to plan and execute operations for the Hospital at Home units, including administrative support, patient care activities, and basic phlebotomy skills. The role will also participate in reporting and monitoring the operational efficiency of the unit(s).

Requirements

• High school diploma or equivalent
• Basic Life Support/BLS certification
• Basic ability to read, write and perform basic four-function math
• Ability to obtain knowledge of medical terminology, basic personal computer skills, and interpersonal skills
