A growing healthcare company is seeking a dedicated Service Coordinator to join their team. In this role, you will be the primary point of contact for patients, ensuring their needs are met through consistent communication via email, phone, and quarterly in-person visitations.
Requirements

- Act as the main point of contact for patients, providing regular communication via email and phone.
- Conduct quarterly in-person visits to patients within your caseload.
- Manage and update patient documentation to ensure compliance and accuracy.
- Respond to patient and caregiver inquiries in a timely and compassionate manner.
