Job Description
A growing healthcare organization in Crown Heights is seeking a full-time Service Coordinator to manage a caseload of up to 25 patients.
Requirements
- Manage case files and coordinate services for up to 25 patients
- Arrange transportation and order medical supplies as needed
- Ensure monthly and annual paperwork is completed accurately and on time
- Conduct quarterly in-person visits with patients to assess and support their needs
- Maintain consistent documentation and meet all compliance standards
- Work collaboratively with other care team members
