Telehealth Heart Failure Nurse

At CopilotIQ+Biofourmis, we're transforming in-home care with AI-driven platform that supports individuals through every stage of their health journey. We're seeking a Telehealth Heart Failure Nurse to monitor and screen incoming alerts, calls and chat communications, and work closely with the Provider Team to enhance health care outcomes.

Requirements

• Current, active, unrestricted LVN license in California
• Minimum 2 years hands-on nursing experience
• Telehealth and chronic care management experience
• Fluency in English and Spanish
• Experience with patient phone triage
• Ability to learn and proficiently use multiple EMRs and communication applications
