Primary Care Physician in Homes and Communities

We are looking for a Primary Care Physician to deliver comprehensive primary care at home and senior communities in San Antonio, Texas. The ideal candidate will be a team player who can build trusted relationships with patients and their care teams, and drive superior patient outcomes.

Requirements

• Appropriate licensure in the state of Texas
• Bilingual preferred but not required
• Ability to commute to assigned assisted living communities
• Ability to travel to Denver market for initial and ongoing training
