Job Description
Breaking Ground is seeking a Case Manager, Transitional Housing to provide case management services to people experiencing homelessness, some with mental illness and/or substance use disorders, to assist them in transitioning to permanent housing.
Requirements
- Provide direct services to a caseload of approximately 15 clients
- Conduct psychosocial evaluations
- Connect clients to psychiatric services
- Obtain identification, public benefits, and other applicable income sources for clients
