Requirements
- Conduct detailed admission assessments for patients entering rehabilitation
- Collaborate with medical, nursing, and allied health teams to support seamless care transitions
- Provide education to referrers, patients, and families on rehabilitation goals and processes
- Liaise with external providers to ensure continuity of care
- Work flexibly across additional days and shifts as required
