Job Description
We are looking for a Medical Case Manager II Licensed to coordinate and deliver comprehensive case management services to clients impacted by HIV, Hepatitis C, STI's, and other infectious diseases.
Requirements
- Master's degree in counseling, Social Work, or related field and current licensure as LMHC or LCSW or associate or bachelor's degree in nursing with active RN or LPN license in good standing
- Three years of HIV prevention/intervention experience
- Must be able to pass a Level I and Level II Background check as required
