Community Liaison PACE
High school diploma, some college required, bachelor's degree in marketing or healthcare preferred. 4-5 years of experience in marketing or healthcare relationship management
Receptionist - PACE
High school diploma or equivalent, one year of experience as a receptionist, typing skills of 35-45 wpm
Clinic RN - PACE
BSN degree, 1+ years of experience working with the frail and elderly population, valid RN License, and valid Colorado Driver’s License
LPN PACE
Graduate of an accredited school of practical nursing, one year of experience in hospice, palliative care and/or home health, medical/surgical, public health, oncology, acute care or home health
Eligibility & Financial Determination Specialist - PACE
High school diploma, 2 years' experience in Medicaid eligibility, financial determination, or benefits coordination, and a valid Colorado driver's license
Care Plan Coordinator PACE
Associate's degree in healthcare, administration, or a related field, one or more years of experience in care coordination, case management, or a highly administrative role, and a valid Colorado driver's license
Home Care RN PACE
BSN preferred. 1 year of professional experience, minimum 1 year experience working with frail and elderly population, home care or geriatric experience
