Job Description
Colorado PACE has an immediate opening for a Home Care Registered Nurse to assess, plan, implement and evaluate care provided in the patient's place of residence. The nurse will collaborate with the interdisciplinary team to meet patients' needs and maintain continuity of care between disciplines, programs, services and with other organizations as needed.
Requirements
- Graduate of an accredited school of nursing; BSN preferred.
- One year of professional experience required, minimum one year experience working with the frail and elderly population, home care or geriatric experience.
- Valid RN License. Valid Colorado Driver's License and proof of Auto Insurance in accordance with state statute.
