Job Description
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our home health care team as a Case Manager. The Home Health RN Case Manager will provide high-quality nursing care and case management services to patients in their homes, coordinating their treatment plans and collaborating with physicians, caregivers, and other healthcare professionals.
Requirements
- Associate's degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited nursing program.
- Current and valid RN license in the state of Indiana.
- Minimum of 2 years of nursing experience, with at least 1 year of experience in home health care or case management preferred.
