Job Description
To promote health and independence by supporting clients/patients with activities of daily living, communicating and involving other members of the multi-disciplinary team in their care to meet planned care goals and contribute to the safeguarding of those receiving care while respecting their rights to be autonomous in their decision making.
Requirements
- Deliver care in a flexible manner across a variety of health and social care settings as directed by the Team Lead, Registered Nurse, Registered Manager.
- Adhere to the seven principles of the Code of Practice for Health and Social Care Support Workers in Jersey.
- Undertake a range of specific tasks following assessment and delegation by a Registered Nurse to be outlined within the clients/patients individual plan of care.
- Work in a holistic way to support clients/patients to achieve his or her physical and psychosocial needs.
- Respect the right to an individual's autonomy and act in the best interests to safeguard those requiring and receiving care by obtaining informed consent for that care and/or interventions.
