
Job description
The Community Based Care Manager collaborates with members of an inter-disciplinary care team, providers, and community organizations to improve quality and meet the needs of individuals through culturally competent delivery of care and coordination of services.
Facilitate communication, coordinate care, and assist members in creating and evaluating person-centered care plans to prioritize and address behavioral, physical, and social determinants of health needs.
This role requires strong communication and coordination skills, as well as the ability to work with diverse populations and navigate complex healthcare systems.
