
Job description
The Community Based Care Manager collaborates with members of an inter-disciplinary care team, providers, community and faith-based organizations to improve quality and meet the needs of the individual, natural supports and the population through culturally competent delivery of care and coordination of services and supports.
Engage members and their natural support systems, facilitate inter-disciplinary care team meetings, develop and update person-centered individualized care plans, identify and manage barriers to achievement of care plan goals, and educate members and natural supports about treatment options and community resources.
This role requires a strong understanding of quality, HEDIS, disease management, and supportive medication reconciliation, as well as excellent communication and organizational skills. The ideal candidate will be a strong advocate for members at all levels of care and have a strong understanding and sensitivity of all cultures and demographic diversity.
