Job Description
The Social Service Designee will assist the LSW/Social Service Director in providing social services to residents and their families, including adjustment process support, need identification, problem solving, and financial assistance.
Requirements
- Provide social services to residents and their families
- Attend Care Conferences as directed by the LSW
- Fulfill basic documentation requirements for social services
- Assist in the discharge planning process
- Assist in the completion of the Integrated Personal Services Assessment and MDS and its components
