Care Concierge, US Remote

Join Carewell's growing care navigation program as a Care Concierge, providing trusted caregiving solutions and support for individuals and families. As a Care Concierge, you will be the steady presence in a patient's healthcare journey, building trust and rapport with patients over time and helping them navigate a complex healthcare system.

Requirements

• Patient-facing healthcare experience in a setting such as medical front office, ambulatory care, primary care, senior care, case management, patient coordination, utilization management, or value-based care programs
• Strong understanding of care coordination principles and the ability to build trust and communicate effectively with older adults and individuals managing serious chronic conditions
• Self-directed and metric-aware with ability to manage your own time and track your caseload proactively
