The Case Management Program provides vital support services to keep frail seniors living safely in their homes and communities.

Requirements

• High School Diploma/GED
• Four years of experience in a social service setting or in providing direct services to individuals or families
• Ability to conduct home/field visits and travel throughout the five boroughs
• Ability to lift up to 10 lbs
