Requirements

• Bachelor's degree in a human services field
• Case management and support to PC clients
• Facilitates groups
• Documents according to regulatory standards
• Carries a caseload
• Interfaces with PC psychiatrist
• Maintains collateral contacts with family members and providers in the community on behalf of clients
