Job Description
Peter Claver Community is seeking a Registered Nurse Case Manager to provide comprehensive care to chronically homeless individuals living with disabling HIV/AIDS. The successful candidate will be responsible for assessing residents' health status, communicating with primary care providers, and working with a team to ensure high-quality care.
Requirements
- California license as a Registered Nurse
- Associate's degree (A.A.) or equivalent from two-year college or technical school
- 2 to 5 years of experience in case management of HIV infected adults
- Good working knowledge of Harm Reduction principles and practices
