Care Specialist

Provides person-centered assistance to clients in the development of life skills and personal goals, coordinates client care, and connects clients with community resources.

Requirements

• Coordinates client care by working closely with a multidisciplinary team
• Teaches and trains clients in activities of daily living
• Connects client with community support systems as determined to be appropriate
