Care Specialist CCBHC

Provides person-centered assistance to clients in the development of life skills and personal goals, coordinates client care with multidisciplinary team, assists clients in connecting to community resources, and provides crisis intervention/prevention to clients and their families.

Requirements

• Coordinates client care with multidisciplinary team
• Teaches and trains clients in activities of daily living
• Connects clients with community support systems
• Provides telephone and on-site crisis intervention/prevention
