Job Description
Join Centerstone as a Case Manager, providing coaching, strategies, and coordination of health and wellness resources to help patients live their lives more fully. Coordinate services and care for patients, teaching the importance of the connection between physical and mental health.
Requirements
- Bachelor's degree in counseling, psychology, social work, or related field
- Valid driver's license in good standing
- Experience with Electronic Health Record preferred
