Case manager, foster care

Centerstone is seeking a case manager for its foster care program. The case manager will provide support to foster care clients, serve as a link between Centerstone, DCS, foster families, and other services, and conduct in-home visits to manage issues and concerns.

Requirements

• Learns and applies Centerstone and DCS Policies and Procedures
• Attends and participates in CFTM's
• Links clients/families to community resources
• Assists families with medication issues
• Helps achieve all EPSDT and Dental requirements
