Clinical Care Coordinator

The Clinical Care Coordinator is responsible for managing a caseload of clients, documenting and charting in electronic health records, communicating with multidisciplinary teams, educating the community on available programming, and ensuring adequate patient care. They will also review recommendations from patient mental health care providers and coordinate treatment and community resources.
