The Health Coach CCBHC serves as an integral member of the multi-disciplinary team, providing individual, supportive coaching to improve physical health status, coordinating client care with internal/external providers, and promoting the best outcomes for clients.

Requirements

• Provides individual, supportive coaching to improve physical health status for assigned clients
• Coordinates client care with team of internal/external providers
• Provides life skills training, care management, and structured interventions to assist consumers in gaining access to resources and the skills necessary to improve their physical health status, quality of life, and self-management of symptoms
