Job Description
Become a Clinical Care Nurse (RN) and contribute to improving patient outcomes in a clinic-based setting. The role involves supporting safe Transitions of Care, reducing avoidable ED utilization, and driving quality performance.
Requirements
- Associate’s degree in nursing (ADN) or Bachelor’s degree in nursing (BSN)
- Active, unrestricted RN license
- 3+ years' clinical nursing experience
- Proficiency with electronic health records and data analytics tools
- Basic Life Support training
