Home Health Registered Nurse

As a CenterWell Home Health nurse, you will provide personalized, one-on-one care to patients in their homes, developing and implementing care plans, monitoring patients' conditions, and educating patients and their families on disease management, medication, and treatment options.

Requirements

• Diploma, Associate or Bachelor's Degree in Nursing
• Med surg, ICU, ER, or acute experience
• Current and unrestricted Registered Nurse licensure
• Current CPR certification
• Experience collaborating with a team of healthcare professionals
• Valid driver's license, auto insurance and reliable transportation
