Job Description
Provide personalized, one-on-one care to patients in their homes as a Home Health Registered Nurse Case Manager, developing and implementing care plans and educating patients and their families on disease management and treatment options.
Requirements
- Diploma, Associate or Bachelor's Degree in Nursing
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid driver's license, auto insurance and reliable transportation
