The Patient Services Coordinator is responsible for scheduling visits, communicating with field staff, patients, and physicians, and maintaining proper care coordination and continuity of care.

Requirements

• 1 year of home health experience
• Excellent communication skills
• Ability to interact well with a diverse group of individuals
• Strong organizational skills
• Ability to manage and prioritize multiple assignments
