We are looking for a Primary Care Physician to join our team full-time at CenterWell Senior Primary Care. This role is ideal for a compassionate, experienced clinician who excels in a collaborative, patient-centered environment and is committed to improving outcomes for adult and geriatric populations.
Requirements
- MD or DO from an accredited medical school
- Active, unrestricted medical license in the state of practice
- Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine
- 2 years' experience in value-based care or managing high-acuity geriatric patients
