Job Description
We are looking for a clinician leader to design clinical programs that improve outcomes for senior and high-risk patients. You will combine clinical expertise with program development and population health strategy.
Requirements
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA)
- Active, unrestricted U.S. license
- 8+ years of experience across clinical care, operations, or program delivery
- Experience caring for senior, geriatric, or complex patient populations
- Experience designing or leading clinical programs, population health programs, or care models
