Job Description
As a CenterWell Home Health RN Case Manager, you will provide personalized care to patients in their homes, helping them regain independence and manage chronic conditions. You will work closely with a team of clinicians to develop and manage care plans that support recovery and overall well-being.
Requirements
- Diploma, Associate or Bachelor Degree in Nursing
- Minimum of one year nursing experience preferred
- Strong med surg, ICU, ER, acute experience
- Home Health experience a plus
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Strong organizational and communication skills
- Valid driver’s license, auto insurance and reliable transportation
