Patient Transport Driver - LYN Pace Transport/PRN Days

This position is responsible for providing transportation services for Centra PACE/Ground Transport under the direction of the Transport Supervisor(s). The Driver will provide a safe and enriching experience for every individual from door to door.

Requirements

• Providing exceptional door to door care to all PACE Participants & Centra Patients
• Communicating all participant/patient changes & needs to direct supervisor
• Performing pre & post vehicle safety inspections, assuring vehicle is in proper operational condition and reporting any maintenance issues to immediate supervisor
• Maintaining cleanliness of interior & exterior of Centra vehicle
• Following scheduled transports for PACE participants or patients & other potential trips communicated by direct supervisor
• Maintaining all necessary records/documentation regarding participants/patients & vehicles
• Communicating frequent changes to direct supervisor
• Performing other duties as assigned
