Job Description
The Physical Therapist provides outpatient rehabilitation services in an in-home setting for the older adult population, assessing patient needs and developing individualized physical therapy programs to facilitate rehabilitation.
Requirements
- Valid Physical Therapy license in the state(s) of practice or proof of eligibility
- Master’s degree in Physical Therapy or a related field, or a Bachelor’s degree in Physical Therapy with five years of experience
- Valid driver’s license and reliable transportation
- Strong organizational and time management skills
- Effective verbal and written communication skills
