Job Description
The PRN Nurse Case Manager will facilitate optimum health care for clients, triage and collaborate with outside healthcare providers, and provide skilled health care to clients while addressing, referring and clarifying medical concerns.
Requirements
- RN licensed to practice in the state of Colorado
- Bachelors of Science in Nursing degree strongly preferred
- Current CPR card required
- At least two years relevant work experience required
- Walk-In Crisis team – at least three years relevant work experience required
