Director of Population Health

We're rebuilding the system for cardiology, creating a world where outcomes, not volume, define success. We're looking for a Director of Population Health to uncover opportunities through data and turn insights into measurable clinical action.

Requirements

• 5+ years in population health, medical economics, managed care, or healthcare analytics.
• Strong understanding of claims data, cost/utilization modeling, and quality measurement frameworks (e.g., HEDIS, Stars, MLR).
• Proven ability to translate data insights into operational programs.
• Experience partnering with clinicians and care teams to drive performance.
• Excellent communicator who can distill complexity into clarity for diverse stakeholders.
• Passion for transforming cardiovascular outcomes through technology and systems thinking.
