Field RN Case Manager Wednesday - Saturday 4/10's Cornerstone Hospice

We're seeking a Field RN Case Manager to work with our team, providing outstanding patient care and customer service. The role involves assessing patient needs, coordinating the Plan of Care, and providing palliative and supportive care to patients and their families.

Requirements

• Current license as RN in the state where the employee will be working
• Minimum of one (1) year nursing experience; hospice or hospital experience preferred
• Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
