Requirements

• Review patient intake assessments/charts and conduct telehealth evaluations.
• Complete, certify, and sign Short-term disability forms.
• Coordinate with clinicians and care coordinators.
• Maintain HIPAA and state/federal compliance.
• Provide direct patient care and education.
