Job Description
Chartiers Center is seeking a Service Coordinator/Re-Entry Specialist to provide services for individuals experiencing homelessness with mental health issues and justice system involvement, including housing referrals, benefits, and mental health/physical health care.
Requirements
- Collaboration with inpatient units, emergency departments, jails, and the legal system
- Provide services in non-traditional settings
- Serve as a monitor and clinical bridge to implement necessary strategies for individuals served
