Job Description
The Outreach Worker will work with a multidisciplinary team to provide direct services to individuals with mental health diagnoses in Holyoke, Massachusetts. The role involves engaging persons served to maintain their physical and mental health, supporting their activities of daily living, and providing opportunities for independence, rehabilitation, and recovery.
Requirements
- College degree in human services or related discipline
- Strong written, verbal, and interpersonal skills
- Empathy for others
- Ability to work well within a team model and independently

- Vehicle for work use and current driver's license
