We are hiring a Care Case Manager in San Diego who can create unique care plans, manage care, and provide feedback to clients. The ideal candidate has a strong communication and problem-solving skills, and is willing to work one weekend a month as backup on call.

Requirements

• Upbeat Personality
• Master in Social Work, Nursing or Relevant Field (preferred)
• Industry Experience (preferred)
• Strong Computer Skills
• Strong Organizational & Communication Skills
• Pass Live Scan Fingerprint Clearance (LIC 9163)
• Valid driver’s license and proof of insurance
• Cell phone with Internet Access
• Proof of Negative TB results
• Willing to Work One Weekend a Month as Back-Up On Call
