Job Description
The Registered Nurse, Community Living provides clinical care and case management to individuals with medical fragility, Intellectual and Developmental Disabilities (IDD) and Mental Health (MH) diagnoses. The Nurse utilizes clinical and case management skills to ensure individuals receive high-quality care.
Requirements
- Active, valid licensure as a Registered Nurse (RN) in the Commonwealth of Pennsylvania
- Prior experience with Community Living Arrangements and case management
- Three (3) years’ experience working with individuals living with multiple disabilities
- Excellent judgment with the ability to independently solve problems and make decisions with little direct supervision
- Experience working with feeding tubes, colostomies, tracheotomies, catheter, and wound care strongly preferred
