Job Description
The Family Team Conference Coordinator will facilitate collaboration between families, the agency, and the Administration for Children’s Services (ACS). The Coordinator will coordinate and facilitate Family Team Conferences, ensuring effective communication between the agency and ACS, and supporting timely documentation and follow-up.
Requirements
- Coordinate meetings and facilitate Family Team Conferences
- Act as the liaison between the agency and ACS
- Document all outcomes from the Family Team Conferences
- Assess and manage cases
- Compile monthly statistics and other reports
