Job Description
We are seeking a Health Coach to work with members with various chronic conditions, promoting self-management education and coaching, and collaborating with healthcare teams. The ideal candidate will have a bachelor's degree in a relevant field, a current license in the state of Washington, and experience in disease management, diabetes education, or health coaching. The Health Coach will provide disease-specific care management interventions, assess members, and coordinate care activities to ensure continuity.
