Job Description
Circadia Health, an AI-powered remote patient monitoring company, is seeking a Nurse Practitioner (NP) to lead proactive care efforts across two skilled nursing facilities in New Jersey. The NP will conduct patient assessments, prescribe treatments, develop care plans, and collaborate with facility teams, leveraging Circadia’s AI platform to identify and manage at-risk patients and enhance patient outcomes. This role combines hands-on care with data-driven insight.
Requirements
- Certified and licensed as a Nurse Practitioner in NJ
- Must have an active license and DEA #
- Medicare participation and ability to have company bill for services on your behalf
- Certification from ANCC (or equivalent) as an Adult, Family, Geriatric and/or Acute Nurse Practitioner
- Current NJ Driver’s License
- Comfort using technology such as Google Suite, multiple EMRs, Slack, AI transcription tools
- Ability to thrive in a fast-paced environment
- Comfortable in an adaptive and evolving environment
- English required, Spanish preferred
